NEOLA Camp Staff and Volunteers
This page includes links to forms needed by NEOLA volunteer and paid staff. We are so thankful for the gifts of your time and talents!
Because the safety of everyone in camp is our first priority, all staff are required to complete an application and screening process. There are four components of our Child Abuse Prevention practice. All staff:
- submit a completed Camp Staff or Returning Camp Staff application (staff who have served within the past 12 months in NEOLA Camp Ministries may submit a Returning Staff application);
- submit a Missouri DHSS Family Care Safety Registry (FCSR) form (staff over 18 years of age), along with a copy of the applicant's social security card or driver's license (if social security # appears on the license). Junior staff sign a Youth Covenant when they arrive at camp;
- submit personal references (mailed directly to NEOLA by referring individuals);
- submit a certificate of completion of the online BSA (Boy Scouts of America) Youth Protection Training.
Links to forms for items 1-3 are below, in both Adobe and MS Word format. You may download the forms and complete them on your computer. Since all forms must be signed, they need to be either mailed to the NEOLA Registrar at P.O. Box 10230, Columbia, MO 65205 or faxed to 573-442-1925.

Please also complete the Staff Brief Health History and bring it with you to camp.
